Navigating the Kentucky BioTech Facility
The mission begins with Case and Marshall inside a toxic Kentucky BioTech facility. An elevator malfunction leads to a gas mask breakage and subsequent hallucinations. After the cutscene, locate a red-lit, locked door. Use a hatchet found on a mannequin to force it open. Proceed through the hallway, up the stairs, and to the central elevator.
Activating the elevator triggers a zombie encounter (hallucination). Defeat them with the hatchet. A phone call directs you to the biotechnology room, requiring four director's cards (Red, Green, Blue, Yellow). You'll receive a map to the Yellow card.
Acquiring the Yellow Card and Grappling Hook
Follow the map to a yellow staircase. Solve the computer puzzle ("Access" and "Lift") in the Director's office to access the A.C.R. room. Eliminate more zombies. The yellow card is held by a mannequin which transforms into an abomination upon approach. Before engaging, gather armor, weapons, and the crucial grappling hook from the surrounding area.
Use tactical explosives (C4 or grenades) to efficiently defeat the abomination and its zombie horde. Retrieve the Yellow Card.
Securing the Green Card
Use the grappling hook to ascend to the main facility. Next, obtain the Green Card from the Administration Facility. Grapple to the facility from the security desk. Answer the ringing phone; you need to locate four documents and place them in the file display area.
While collecting (documents are on a corner desk, near a round table, on a small central table, and in the cafe), keep moving to avoid the pursuing mannequins which freeze when you turn. After placing the documents, grapple the attacking red mannequin repeatedly until it transforms into a Mangler Zombie. Defeat it to get the Green Card.
Obtaining the Blue Card
Grapple across to the Joint Projects Facility. Answer the phone, locate the glass chamber with camera stands, and retrieve the Blue Card. Defeat the Mimic that appears. The Mimic can disappear; shoot moving objects to force it to reappear.
Retrieving the Red Card
Go to the East Wing (marked on your map), following the red carpets to a room with water, a console, and a Mangler. Interact with the console to reveal the Red Card. Use the grappling hook to reach the upper area, swim through the red tunnel, and climb the ladders. Eliminate the zombies and use the blacklight to unlock a door. Solve the drain puzzle (three switches) within 25 seconds.
After draining the water, follow the Mangler's escape route and defeat it and the accompanying zombies to obtain the Red Card.
Confronting the Disciple
Return to the security desk, insert all four cards, and ride the elevator. Fight off any pursuing zombies. Answering the phone in the BioTech room initiates a final confrontation with countless zombies and the Disciple. The ensuing cinematic reveals the entire encounter as a hallucination.
